the Rackham Graduate School application process.

On-Line, Via the Web: The University strongly encourages you to apply
online; there are advantages both to the student
and the processing staff.
OR
Complete the PDF Version of the Application: You may also download
the application and print a paper application.
OR
Request an Application on CD: You can complete the Program
Information Request Form and request an application disk or submit your request
by contacting
us (umnursing@umich.edu).
If you complete a paper version, please allow seven to ten days (U.S. Mail)
for application materials to reach the University prior to the published deadline.
The Rackham application and fee must be submitted to the Horace H. Rackham
Graduate School. The Departmental copy of the application and all supporting
documentation required by the
School
of Nursing must be submitted to the Office of Doctoral Studies.

Apply! Rackham Program Graduates and Current Rackham Students
Rackham
students (past or present), must download, print and complete the Rackham
Application for Readmission, Change of Program, etc., -- there is no
fee required. Please submit this form and all supporting documentation
