Community Family Health Center
The Community Family Health Center (CFHC) is a community-based primary health
center where you and your family will receive complete individualized holistic
primary health care in a relaxed professional setting. Care is provided by
nurse practitioners working collaboratively with physicians and other health
care providers.

Contributions
Contributions from individual donors have enabled both North Campus Family
Health Service and Community Family Health Center to extend services and
develop new programs. For example, the generosity of individual contributors
has enabled us to offer prescription assistance to indigent patients, and
to purchase equipment for our Outreach Clinic.
If you wish to make a tax-deductible donation to support the work of the
clinics, you may mail it directly to the attention of Carol Zastrow, Clinic
Manager at the clinic location. You will receive an acknowledgement via return
mail to use for tax purposes.
