Personal Action Toward Health (PATH)

PATH is a program designed for high risk Asian Americans to adopt a healthier lifestyle by taking personal actions. Asian Americans who are at high risk of or being diagnosed of high blood pressure, cholesterol, blood sugar or low bone density, or cancer survivor and other family history related conditions can meet at a neighborhood community center to discuss, learn and take action toward better management of these chronic diseases which will lead to improved health.

HAAP coordinators recruit these eligible individuals at various locations and events, for example, after a health fair, church gathering, or social events. HAAP coordinators will then arrange the times and location to hold the 6-week PATH workshop at convenient locations in the community.

Community groups with no more than sixteen people will meet with two certified leaders for two and a half hours a week for six weeks. Participants will receive a program book in English, Chinese or Korean and are able to discuss in the language that the group chooses.

The PATH program is designed to benefit the overall health of high risk Asian Americans. Individuals who suffer from chronic conditions should still strive for better health and lead a normal life style as much as possible. Participants in the PATH program can meet others with similar interests and help spread better health to friends and family.
