Compliance Requirements

All undergraduate, masters and DNP students are required to submit U-M School of Nursing compliance documents. Documents must be submitted by August 1st. Failure to provide complete documentation may result in an unexcused absence from the clinical component(s) of a course.

To better serve our students, the School of Nursing partners with CertifiedBackground.com to manage our students' health compliance documents. This management system allows our students to meet the ever-evolving requirements of the clinical agencies where they fulfill the clinical requirements of their coursework.

Information & Instructions for Students

Opening a Certified Background Account

  1. Go to CertifiedBackground.com and click Students.
  2. In the Package Code box, enter the package code for your program. (Undergraduate students use UB58; Graduate students use UB59)
  3. Your package requires you to submit immunization, medical or certification records.  At the end of the order process, you will be prompted to visit a secure web page where you will view additional instructions for uploading your records.

Retrieving Information

  • Once your order is submitted, you will receive a password via email to view the results of your background check. The results will be available in approximately 48-72 hours.
  • To view your result, visit CertifiedBackground.com, enter your password in the area provided on the lower right of the home page, then enter the last four digits of your Social Security Number.
PLEASE NOTE: Do not send compliance documents to the School of Nursing. All compliance records must be uploaded into your Certified Background online account through their record keeping mechanism, Magnus Health Portal (magnushealthportal.com). Records can also be faxed or sent via mail to the agency; however, when faxing or mailing records, be sure to include the HIPAA release form which can be downloaded from your online account.

Clinical Placements Requiring Drug Screening

If you are doing a clinical rotation at any of the following sites, you will be required to complete a drug screening. This must be completed before starting your clinical rotation.

Discounted Drug Screening for Students

The School of Nursing has negotiated a discounted rate of $25.00 specifically for U-M students. You must show your U-M ID in order to receive this special rate. The testing site is:

They are generally open Monday, Tuesday, Friday, Saturday, and Sunday from 8:30 a.m. to 1:30 p.m. and Wednesday and Thursday from 2-7 p.m. It is recommended that you call before going over to confirm that they are open.
 
Please note that although some agencies only require a 3-panel drug screen, the above lab performs a 10-panel drug screen and because of the discounted price, it is less than what you would pay for a 3-panel drug screen.
 
Since Magnus (to whom you submit your other health/immunization requirements) does not review these special cases, the lab must fax the results to:
