Materials and information have been received from the following health systems and are provided here to assist you with the required process prior to beginning your clinical activity. Each system has different requirements and most of the requirements must be repeated annually. Additional information, if any, will be provided by your clinical faculty member.

NOTE: Students, give a copy of documents to your clinical instructor and keep originals for your records -- it may save you valuable time.
